Raising a Repeated Prescription Error

You collect your prescriptions from Collineau Pharmacy in Quebec's Eastern Townships. Last month, the pharmacy supplied your prescription with the wrong dosage on the label. At the counter, you told Benoît, the pharmacy assistant, that the details were correct without checking. You noticed the error at home but put off reporting it. Today, your next refill has the same incorrect dosage. You have not taken any medication from either refill. You feel uncomfortable admitting that you failed to report the first error, but you need the problem resolved. The counter is busy. Pharmacist Mireille can speak privately after a short wait, or Benoît can arrange a private telephone conversation with her later today.
